If you don't wish to order your product on-line by credit or debit card (visit our Products page to place your order on-line), you can print this order form and post it to us along with a cheque made payable to SPDVP.

You can also send us a note rather than print this order form (for example if your browser has difficulty displaying the form below correctly). Just write down the Product Title, Product Code and quantity of each item and send this to us along with your cheque. You may wish to revisit our Products page again to make a note of the details for each item you would like to purchase.
